Acute respiratory distress syndrome (ARDS) is a devastating critical illness characterised by acute inflammatory lung injury, associated with increased pulmonary vascular permeability, increased lung weight, and loss of aerated lung tissue. A personalised approach to the treatment of ARDS patients following the recognition of individual phenotypes is likely to improve treatment decisions in the near future.
